CMS added validated malnutrition screening (MNA-SF, MUST, or equivalent) to the Age-Friendly Hospital Measure Domain 3 requirements in August 2025. Hospitals that did not update their admission protocols are at risk of a domain gap on the CY 2025 attestation.

Hospital Commitment to Health Equity scores from the CY 2024 attestation cycle are now publicly visible on CMS Care Compare as of January 2026. Patients, payers, and accreditors can see your current score. Your CY 2025 submission (due May 15) will update those scores in fall 2026.

PSSM submission now via NHSN — not CMS HQR
The Patient Safety Structural Measure moved to CDC NHSN for submission in 2024. Hospitals attempting to submit via the CMS HQR portal will not find it there. NHSN login credentials are separate from CMS credentials — confirm your facility has an active NHSN OrgID before the submission window opens.
