IQR structural calendar, checklists, and surveyor asks

Planning views for Age-Friendly, PSSM, Maternal Morbidity + DACA. Confirm every date on QualityNet, NHSN, and your accreditor. Not legal or compliance advice.

2026 / 2027 attestation calendar

Illustrative U.S. hospital cadence. HQRP publishes firm open/close dates each payment year. Use this to lock the spring evidence file in the fall; do not treat it as “submit this week.”

Age-Friendly Hospital Measure crosswalk

Submitted in CMS HQR as part of IQR structural / Structural MSR. Same HQR window as Maternal Morbidity and DACA. Not the same portal as PSSM.

4Ms / domainWhat CMS is askingTypical evidenceTypical owner
What Matters · Domain 1Protocol to elicit, document, and update goals, preferences, advance directives, and proxies at admission, pre-procedure, and status change.Named written protocol; EHR fields; training rosterCMO / Care Management
Medication · Domain 2Named framework (Beers, STOPP/START, or equivalent). Reviews on admission, before major procedures, and on status change.Protocol naming the criteria; pharmacy workflow; PIM metricPharmacy + Quality
Mentation + Mobility · Domain 3Validated cognitive, mobility, and malnutrition screens plus structured intervention. Malnutrition added August 2025.Named tools (4AT/MoCA, TUG/SPPB, MNA-SF/MUST); intervention pathwayNursing + Nutrition
Beyond the 4Ms · Domain 4Social isolation, caregiver stress, elder abuse: screen and respond.Protocol; social-work pathway; referral logSocial Work / Case Management
Leadership · Domain 5Named champion or interprofessional committee with a written charge.CMO/CNO designation letter; minutes; board reportCNO / Quality Director

To attest yes, practices must be in place for the majority of older patients. A single admission note does not satisfy the update cadence.

PSSM domain checklist

Patient Safety Structural Measure is per-facility NHSN, not HQR. Confirm OrgID and the active reporting year in NHSN.

  1. Leadership commitment. Governing board treats safety as a core value; serious events to C-suite/board within three business days; culture assessment at least every two years.
  2. Strategic planning & policy. Safety goals in the strategic plan; just-culture policy that distinguishes human error, at-risk, and reckless behavior; FMEA or equivalent.
  3. Culture & learning. Validated culture survey (e.g. AHRQ SOPS) used for unit action; at least four of the CMS Domain 3 Statement D practices (huddles, rounding, PSO, near-miss, trigger tools, etc.).
  4. Accountability & transparency. Internal and (where you claim it) public safety data; named owners; peer benchmark use.
  5. Patient & family engagement. PFAC that reflects the population; family in safety governance; record access with language support.

Maternal Morbidity Structural Measure

One official two-part question in HQR. Both parts must be true to attest Yes. Hospitals that do not provide inpatient labor/delivery still submit N/A — the IPPS Measure Exception form cannot be used instead. A prior Yes is how CMS awards the Birthing-Friendly designation on Care Compare.

Data Accuracy and Completeness Acknowledgement (DACA)

DACA is not a clinical measure. It is the annual HQR acknowledgement that Hospital IQR Program data for the reporting year are accurate and complete to the best of the authorized user’s knowledge. Same typical window as Age-Friendly and Maternal Morbidity: April 1–May 15. Confirm dates on QualityNet.

  1. Named QualityNet Security Official (and a backup) who can sign in HQR.
  2. Age-Friendly file reviewed against dated evidence before anyone clicks acknowledge.
  3. Maternal Morbidity response selected (Yes, No, or N/A) with the collaborative/bundle file or service-scope note.
  4. PSSM tracked in NHSN — signing DACA does not submit PSSM.
  5. Window on the quality calendar with a named owner. Nothing can be added after the deadline.

What surveyors ask for

A one-pager Quality can forward. If you cannot produce the artifact in the room, do not attest yes.

  • The named written protocol, not “we do this clinically.”
  • Proof it applies to the majority of in-scope patients (audit, EHR report, or workflow monitor).
  • Dated committee minutes, training completion, and the last review of the policy.
  • A named owner who can walk the file (not a shared inbox).
  • For Age-Friendly Domain 3: the malnutrition tool name added in the August 2025 CMS update.
  • For PSSM: NHSN confirmation, not an HQR screenshot.
  • For Maternal Morbidity: the named collaborative and implemented bundle file, or the N/A service-scope note.
  • For DACA: the authorized QualityNet user who will sign, after Age-Friendly and Maternal files are reviewed.

Self-reported answers in a scan are intake. The working file is dated evidence with owners.
